At Whole-Self Wellness, we believe that your mind, body, spirit, and relationships each play a role in your overall health.
If you feel like you're not reaching the ideal in any (or all) of these categories, it can be difficult to identify what's wrong or where to begin in the healing process.

Can you answer "Yes" to any of the following?
I feel stressed 3-5 times per week.
I feel down or depressed 3-5 times per week.
I feel worried or anxious 3-5 times per week.
I have feelings of guilt 3-5 times per week.
I have thoughts 3-5 times a week that negatively impact my mood or motivation.
I struggle falling or staying asleep.
I wake up feeling unrested or lack motivation throughout the day.
I have headaches, muscle aches, or other aches and pains 3-5 times per week.
I have multiple doctors and/or have repeated appointments.
I have crave salty, sweet, or grain snacks frequently.

Some questions have a point value assigned. There is no grade attached to this assessment, but scoring 3 or higher should be taken as a sign that category of your health requires additional care.
